The word means pathetic, not cute. Hello Kitty is the spokes person of 'kawaii'. She is a infantile female character that is handless, mouth-less and expressionless. These aspects of her appearance represent her useless/helplessness.

Jenn here. I think it is one of those slurs that can have positive or neutral meanings in a different context. Much like troll and tr***. I had a Japanese headmate and he never told me this, also, in my past life, I was frequently called this by my human family. My former family loved me and certainly did not think I was pathetic.
